POLICY OVERVIEW

The University of Texas Health Science Center at Houston (UTHSC-H) is in compliance with the Family Educational Rights and Privacy Act (FERPA) of 1974, which protects the privacy of educational records and establishes the rights of students to inspect and review their educational records. Students have the right to file complaints with the FERPA Office concerning alleged failures by the institution to comply with the Act. Copies of the Act are available in the Registrar's Office and the Dean's Office of each school. Also available is a statement setting forth the type of records maintained by the institution, the name and locations of persons responsible for maintaining them, and the procedure established to access, review, and expunge those records. (Refer to Appendix C-Guidelines for a copy of the FERPA guidelines.)

PROCEDURE

I. Names

The student's full, legal name will be used on all permanent academic records, so that there is a consistent use of the full, legal name on (a)the permanent academic records, (b)certifications, and (c)diplomas.

The full, legal name will initially be obtained via the application for admission.

The student is responsible for notifying the Registrar's Office of any name change after admission.

II. Diplomas and Certificates

One original diploma or certificate will be issued to each student upon his or her graduation. The cost of the diploma or certificate is included in the graduation fee.

If at any time a graduate desires a copy of a diploma or a certificate, he or she may place the order with the Registrar with payment for the cost of the purchase. Each copy of a diploma and certificate will be acknowledged by the citation "Duplicate," which will be printed in small letters on the lower portion of the credential.

III. Transcripts

A student may obtain a transcript of his or her UTHSC-H record by submitting a written and signed request. There is a fee for each transcript. No transcripts will be issued showing only a portion of the student's academic record.

Students who owe debts to the UTHSC-H may have their official transcripts withheld until the debts are paid.
